The same thing happened to me; I had to run the installer twice to fix the problem. All of this was in version 1.2.1.0.
@Theremino @Shepard-N7 I installed version 1.2.1.0 as update over a previous version and also as a fresh installation. In both cases FxSound.exe got copied correctly.
Are you trying to install over a Theremino 2.x version?

That is correct. I installed it over version 2.0.4 and encountered that issue.
I thought that installing it over 2.0.4 might cause some errors, but I took the risk and did it anyway.
At the moment, everything is working fine (v1.2.1.0) with nothing negative to report aside from what was mentioned earlier.

Just wanted to echo @bvijay in our thanks to you @Theremino for the work you’ve done to help improve FxSound. And I also wanted to shout out @bvijay for all the work he did to integrate the changes that @Theremino has offered.
While we won’t be integrating all of @Theremino ‘s changes in one go, and we will be making some minor deviations here and there in ways we think will improve the user experience, we will be using the core of what you’ve created, @Theremino .
