Can you provide the details of the application version, Windows platform (x64/ARM64) and version.
If you recently updated FxSound, it will show it blank like that, so you need to delete it and add it again.

I observed this issue on ARM64 and fixed it in v1.2.10.0. But on x64 I am not able to replicate this problem.
Please try v1.2.10.0 and if you still see a blank icon, share the steps to replicate the issue.
